Hallucination
When people say an AI "hallucinates," they mean it stated something confidently that is not true. It invented a statistic, cited a report that does not exist, or gave you a number that does not match your data. This happens when the AI does not have access to the right information and fills the gap with plausible-sounding guesses. The fix is not a better model — it is a better data foundation. When your AI agent is connected to a governed dataset with a semantic layer that defines every metric, it does not need to guess. It queries, retrieves, and cites. An AI grounded in your actual data cannot hallucinate your own numbers.

Your billing manager asks the AI: 'What was our denial rate for Q3?' The AI responds: '7.4%.' Your billing manager knows it was 11.2% because she ran the report yesterday. But your CFO, who does not have that context, might take the 7.4% at face value and present it to the board. That is the real danger of hallucination — it is not that the AI is wrong, it is that the wrong answer looks exactly like a right one. There is no hedge, no confidence score, no flashing red light.
The trap is trying to solve hallucination with a better model. Upgrading from one LLM to another might reduce hallucination rates from 8% to 4%, but 4% wrong answers delivered with 100% confidence is still a business risk. The actual fix is architectural: make the AI retrieve verified data before answering, cite its sources, and explicitly say 'I do not have enough data to answer this' when it does not. A well-built system makes hallucination nearly impossible for factual queries because the AI is reading from your database, not generating from memory.
Questions to ask any vendor: When the AI answers a data question, can it show the exact query it ran and the exact result it got? What does the system do when it does not have enough data to answer — does it say so or fill in the blank? Can you show us an example where the AI refused to answer because the data was insufficient?
